  • He investigated the failure of a motorcycle engine and determined that the engine failed due to oil deprivation, despite the observation that the crankcase was full of clean oil. Inasmuch as the bearings had melted he conluded the oil was added post accident. The accident resulted in severe personal injury.
  • He investigated the cause of a residential fire and determined that it was caused by a faulty electrical connection box in the kitchen, which proved correct, despite the claims of the Pennsylvania fire inspectors that the fires started in the living room, which was little damaged. The accident resulted in no personal injury.
  • He investigated the cause of a collision between a private automobile and a supermarket building, which indicated that the steering mechanism had failed. The failure was due to poor design of the steering mechanism, which allowed the linear rack gear to inadvertently rotate, preventing its full contact with the pinion. The accident resulted in little personal injury.
  • He investigated the collapse of an outdoor table on a fast-food restaurant patio. The plastic table-top cover was insufficient to protect the wood fiber base from rain contact which caused deterioration of the table base. As a result the table top flipping off. The accident resulted in severe personal injury.
  • He investigated the failure of a office swivel chair. Rather than being a catastrophic failure offering little warning he found that the chair had been deteriorating over a considerable period with obvious notice. The ultimate failure was due to poor design. The accident resulted in moderate personal injury.
